I furthermore certifiy that I am in good health and able to participate in all programs and activities. If medical attention is required for illness or injury during training, I give permission for such care. I understand that all injuries that may be sustained during training shall be my responsibility. All refunds are subject to a $30 processing fee and must be requested prior to the start of camp.
